What is Hair
Transplantation?
Hair transplantation is a surgical procedure
in which hair from the back and sides of the scalp is moved to
the bald areas.
When Can a Hair Transplant Be Done?
A Hair Transplant can be done in the early stages of hair
loss when the hair is just beginning to thin, as well as in
the later stages when baldness is more apparent.
In What Type of Baldness Has This Treatment Been
Helpful? It is most commonly used to treat male pattern
baldness. Hair Transplantation has, however, been
performed with excellent results in the treatment of genetic
hair loss in women, as well as in scars resulting from face
lifts and other facial surgery, burns, accidents, or as a
result of various hair loss diseases. This surgery is also
done to help replace thinning eyebrows and eyelashes.
How Long Will the Transplanted Hair
Last?
Transplanted hair should grow normally in the
recipient area as long as it would in the donor site from which
it was taken.
Do All Transplanted Hair Grafts Grow?
In almost all cases, the vast majority of
transplanted grafts should yield growing hair. Pre and
postoperative instructions are provided to minimize the risk of
graft injury and subsequent compromised growth. It is
important to realize that the number of hairs growing from each
graft will vary, depending on the density of the hair at the
donor site and on the size of the graft selected.
How is this Surgical Hair Restoration
Procedure Performed?
Hair Transplantation is a long office
procedure, which requires only local anesthetic. There is
initially some discomfort associated with the administration of
the anesthesia, but this is of very short duration. We have
developed techniques to reduce this discomfort, and most
patients tolerate this brief period very well.
The donor area (the back or sides of the scalp) as well as
the recipient area (the bald or thinning area) are made numb
with the local anesthetic. Up until several years ago,
round grafts (plugs) were always used throughout the hair
transplant process. The desired number of round grafts were
removed from the donor area and then transferred to the bald
area. Today these larger grafts have been mostly replaced
by smaller grafts (follicular units, micro, mini or slit
grafts), since these grafts produce a much more natural
transition from thinning or baldness to increased hair
growth. In most cases where very small grafts are used,
this surgery can be accomplished almost undetectably.
Specific recommendations are discussed at a personal
consultation.
When smaller grafts are to be transplanted, the
following technique is used:
The hair to be transplanted is trimmed close to the scalp,
and the desired amount of donor skin and hair are then
removed. This donor site is then sutured closed, resulting
in a fine scar which is only detectable on close
inspection. The donor hair-bearing skin is then divided
into very small pieces which generally contain 1-3 hairs.
These grafts are then rapidly inserted into small openings
made in the recipient area.
When there is a Large Area of Baldness,
is it Possible to cover the Entire Bald Area?
This depends on the size of the bald area as well as on the
donor area. When there is extensive baldness, a procedure
called a scalp reduction may be useful in order to decrease
the size of the bald area. Subsequently, grafting is done
as described above.
How Many Sessions are required in a Hair
Transplant?
The number of sessions varies, and will depend on the:
Area of scalp treated
Number and size of grafts used
Density of hair desired by the patient
Individual characteristics such as coarseness or
fineness of hair
Current stage of hair loss and future rate of hair loss
Timeframe in which patient wishes to replace lost hair
Is There a Minimum or Maximum Time
Period that is Allowed Between Individual Hair Replacement
Surgery Sessions?
Minimum time intervals will depend upon individual
circumstances. However, there is no maximum interval
between sessions. Certainly, the shorter the intervals
between sessions the more rapidly the hair transplant is
completed.
How long does it take for Transplanted
Hair to Grow?
The transplanted hair generally sheds within the first 2-3
weeks after the procedure and new hair begins to grow
approximately 2-4 months later. The hair then grows at the
normal rate of 1/2 inch each month. The transition from
thinning or baldness to greater fullness of hair is very
gradual.
What is a Scalp Reduction?
A scalp reduction is a procedure, which is performed under
local anesthetic in which a large bald area of skin is
removed. This leaves a scar on the scalp which must later
be camouflaged by grafting directly into the scar.
What is Scalp Flap Surgery?
Scalp flap surgery is a procedure in which an entire segment
of hair-bearing scalp is transferred into a bald area. This
is a relatively uncommon surgery and few surgeons perform
it. The risks include improper hair direction at the
hairline, as well as the more serious risk of partial or
complete loss of survival of the flap and the hair within
it.
Why Have I Seen Hair Transplants That
Have a Pluggy Look?
In the past, hair transplant surgery often resulted in a
very artificial look sometimes referred to as a pluggy,
doll's head or corn row look. This was
usually seen in the intermediate stages between hair
transplant sessions, or if a person decided not to complete
the
required number of surgeries. With the advent of
the tiny grafts being used today, the result can be very natural
even after a single surgery.
Are Lasers Used in Hair Transplantation?
Laser hair transplantation has been used with
some success. To date, results with lasers have been shown
to be inferior to those obtained with traditional non-laser hair

Surgery?
Hair transplantation is an extremely safe out-patient
procedure that is generally without significant risks or
complications. However, as is the case in any surgery,
there are risks, and these are always presented and
discussed in detail at your personal consultation with the
doctor.
What Type of Preparation is required

All patients must have normal results to standard
pre-operative blood tests prior to confirming a surgery date

Definition of FUE
Follicular Unit Extraction (FUE) is the most scientific &
advanced technique of hair restoration. In this procedure:
Hair follicles are extracted individually from the back side
of the head, beard or other body parts with the help of special
micropunches (size varying from 0.7mm - 1.3mm).
Implanted in the bald area of scalp, beard, moustache,
eyebrows or chest.

Donor Area
The hair in the scalp are arranged in the form of groups called
grafts. Each graft consists of 1-5 hair. These grafts are removed
from the permanent zone of back and sides of the head. The hair in
these grafts do not have a receptor in the area of the hair root and
do not respond to the male hormone, testosterone. Also other sources
of hair include: beard, chest, abdomen, legs.

These hair are then implanted in the recipient area.
FUE Hair Transplant
WHAT IS AN FUE HAIR TRANSPLANT?
Hair transplantation originated in Japan in the early
1900s and has evolved greatly over the past century. In the
1980s, as part of this evolution, came the advent of
follicular unit extraction (FUE). FUE, wherein the
practitioner extracts follicular units (natural groupings of
1 to 4 hairs) one at a time, is now the ideal method of
extraction for hair transplantation.
FUE hair transplant is a no-stitch procedure that
involves implanting the grafted follicular units into the
balding recipient area. While FUE hair transplantation
requires the practitioner to possess a higher skill set in
order to perform the procedure successfully, this technique
continually yields the most successful outcomes in hair
density, natural-looking hairlines, and minimized scarring.
FUE hair transplantation is generally pursued for the
treatment of androgenic alopecia, also known as pattern
baldness. This genetic hair loss condition results in
balding of the crown, front, hairline, and temple points.
But with the innovations of the FUE hair transplant, it is
nearly impossible to tell a natural head of hair from one
that has undergone hair transplantation.

PREPARING FOR AN FUE HAIR TRANSPLANT
Once you’ve had a one on one consultation with your
physician and decided to proceed, you will be given the
necessary instructions to prepare yourself for your day of
surgery. Some of these instructions may be tailored to your
particular needs. Put the instructions into place whenever
specified and follow them carefully. On the morning of your
surgery, you should have a bountiful breakfast and arrive
with a thoroughly shampooed scalp. Upon arrival at the
clinic, “before” photographs will be taken of you. The donor
area, being the back of the head in most cases, will then be
shaved and prepared for surgery.

WHAT HAPPENS DURING AN FUE HAIR TRANSPLANT
PROCEDURE?
First, the patient is made to lie face down on the
operating table. Local anesthesia will be administered to
the donor area, as well as intramuscular hypnotics for the
patient to ingest orally, for the purpose of relaxation.
Your surgeon will then begin to extract carefully
selected follicular units from the donor area. The surgeon
will use a micropunching tool, needle devices, and
magnification for the extraction process. (Vacuum-based mass
production machines may compromise your results, and robots
lack the necessary human touch that contributes to an
optimal procedure. These mass production line cookie cutter
FUE machines are at their primitive stages of development
with much room for improvement. They are no substitute for
an experienced FUE surgeon with a verifiable portfolio of
patient results.) Each follicle extracted may consist of
anywhere from one to four, or even five hairs. The extracted
grafts are then preserved in a holding solution. This part
of the surgery is the most laborious and time-consuming,
since unlicensed technicians are legally forbidden to
perform follicular extraction. Your FUE surgeon or an
experienced associate (one who is licensed to perform
surgery by the state) should perform this entire process
himself, to ensure the integrity of each extracted graft.
Body and facial hair are harvested in a similar but more
nuanced manner by experienced practitioners, skilled in
advanced forms of FUE hair transplant.
After all extractions have been done, the patient can
turn over to sit face up. Many would use this time to listen
to music, or watch a movie or television. Local anesthesia
will be administered to the bald recipient area at this
time. Using customized blades, microscopic slits are made in
this area for implantation of the extracted follicles.
Implantation of the follicles is the step requiring the
highest level of skill, as the resulting design or “look” is
determined by the slit preparation. Hairline creation, which
should have been discussed in detail during your
consultation, happens here. The hairline is the most
creative work in the FUE hair transplant process. The utmost
care is given to produce a natural looking hairline. In
order to achieve this, the follicles containing a single
hair are implanted at the front of the hairline to build a
gradual wall of hair, which is filled in by units containing
two to three hairs to provide the area with density.

AFTER THE FUE HAIR TRANSPLANT PROCEDURE
After you receive detailed instructions from the medical
assistant—which include everything from showering to
exercising—you will be given a prescription for mild
painkillers, antibiotics, and topical medication.
Signs of wounding in the donor area should deplete by day
seven, as well. Typically, the donor area will continue to
heal to the point where scarring is highly unnoticeable.
Scabs in the recipient area will usually fall off within
seven to 14 days. This area will go through a phase of
shedding until the eighth week. Typically, your FUE hair
transplant will boast new hair growth by three and a half to
four months, usually maxing out at 12 months. However, for
up to 18 months, the new hair will continue to thicken and
mature, blending in more with the surrounding hair.
WHY CHOOSE FUE HAIR TRANSPLANT OVER DIFFERENT
HAIR RESTORATION OPTIONS?
There are different options for treating hair loss, not
all surgical:
Minoxidil, commonly known as Rogaine, and
finasteride, commonly known as Propecia.
Surgery is the most popular means of permanent hair
restoration with three main techniques at the forefront:
scalp reduction
temporoparieto-occipital (TPO) flaps and other
variations of flaps
Punch graft, minigrafts, and micrografts
Hair transplantation, including follicular unit
strip surgery and FUE hair transplant
Rogaine and Propecia are the two FDA approved,
prescription-based hair restoration drugs on the market.
These drugs must be used continuously in order to stop hair
loss, unlike FUE hair transplantation, which is a permanent
procedure.
Scalp reduction and temporoparieto-occipital flaps, or
TPO flaps, have become obsolete due to consistently botched
results, a high probability of permanent scarring, and the
potential for nerve damage. Also, they are considered hair
restoration surgeries, unlike the FUE hair
transplant, which is transplantation surgery.
The FUE hair transplant is an improvement of the punch
graft technique. Punch grafts are 4mm grafts which result in
what we call “doll’s hair.” Another unsightly outcome
of punch graft surgery is the 4mm round bald marks left
permanently on the back of the head. Minigrafts and
micrografts were developed as an improvement of this method,
but still yield unnatural, pluggy-looking results as
compared to FUE hair transplant.

FUE Hair Transplant vs Strip Surgery:
Follicular unit strip surgery (FUSS) also known as “strip
harvesting,” is considered the traditional method and has
been the recent leader in hair transplant surgery. FUSS
extracts a strip of flesh from the rear and/or sides of the
head, which will be microscopically divided to provide donor
follicles for transplantation. A
disadvantage of FUSS is that it promises a permanent
linear scar to be left as a result. The patient must then
wear a longer hairstyle in order to conceal the scar.
FUE hair transplant does not extract follicular units in
a strip of flesh like FUSS. This means FUE hair
transplant is more labor intensive for practitioners,
requires longer operation time, and is more expensive for
patients. However, consumer demand agrees that the pros of
FUE hair transplant as compared to FUSS far outweigh the
cons. FUE hair transplantation offers:
A less invasive procedure
A quicker recovery time
The possibility to use finer hairs, such as those
found at the nape of the neck, to better refine the
hairline
No linear scarring; only tiny round, cosmetically
negligible marks in the donor area. This also means
patients will be free to wear short hair styles, not
bound to long hair styles for concealing a strip scar.
Less likelihood of permanent nerve damage.
An infinitely larger pool of donor hair, as FUE hair
transplant enables body hair transplantation (BHT)
Body Hair Transplantation:
Furthermore, advanced FUE practitioners such as Dr.
Umar—a pioneer and world leader in this technique—are able
to markedly broaden the pool of donor hair through the
advent of body hair transplantation. BHT allows for the
extraction of donor hair from the beard, chest, stomach,
arms, and legs, with cosmetically negligible scarring.
Patients who lack head donor hair and were therefore
considered poor candidates in the past now join the ranks of
potential FUE hair transplant patients.

GOOD CANDIDATES FOR FUE HAIR TRANSPLANT
Potential candidates for FUE hair transplant using head
hair only are those who do not suffer a total depletion of
scalp donor hair. An FUE clinic that does not practice BHT
would be unable to service a patient without sufficient head
hair remaining in tact.
A clinic that does put body hair transplantation into
practice, such as the Empathy Clinic of Los Angeles, would
be equipped to service patients whose scalp hair loss is so
severe that their head donor hair is not sufficient for
transplantation. This includes FUE hair transplant patients
who:
Suffer from severe pattern baldness
Have excessive scarring from previous surgery
Have endured some sort of accident or burn that has
left them bald
POOR CANDIDATES FOR FUE HAIR TRANSPLANT
While follicular unit extraction using body hair
transplantation has changed the face of hair loss, there
remain some people who still will not qualify for this type
of surgery.
People who lack the supply of good quality head,
body, or beard hair will not be candidates for any type
of FUE hair transplant, even with the use of BHT.
There is a relative contraindication for hair
transplant generally in patients who are too young,
i.e., below the age of 24. This is due to the fact that
a person under the age of 24 who suffers from androgenic
alopecia has likely not finished balding. Also, this age
group may be psychologically and financially unprepared
to grasp the long term implications of hair transplant.
That said, after careful counseling, even young patients
can undergo an FUE hair transplant. Counseling must
fully take into account: future hair loss, total
available donor pool, and the patient’s ability to
countenance future follow-up transplantation (which
could create an unnatural look if not done).
Some People with extremely curly or kinked
follicles, such as most of African descent. These
patients should ideally under go a test procedure to
determine their transection rates and consequently their
suitability for FUE hair transplant.
People with an abnormal tendency to scar, as in scar
hypertrophy, or keloids.
People with bleeding or clotting abnormalities.
People with allergies to local anesthesia.
Would have been a poor FUE hair transplant candidate
without BHT.
FUE hair transplant on a younger patient.
COMPLICATIONS OF FUE HAIR TRANSPLANT
Follicular unit extraction is a safe and minimally
invasive method of hair transplantation. Patients can
minimize their risk by following all pre- and
post-procedural instructions and by selecting an experienced
surgeon. However complications may yet arise even in the
best of circumstances. Factors that increase the chance of
complications of FUE hair transplant could include:
Physiology of the patient
An inexperienced practitioner
A practitioner who values quantity over quality
A practitioner who abdicates his skills to
unlicensed personnel or flawed machines ( eg vacuum
based technologies or robots that lack the human touch)
Unknown and unforeseen complication factors may
arise on rare occasion
WHY FUE TOOLS SUCH AS NEOGRAFT AND ARTAS COULD BE
THE WRONG OPTION
In the opinion of the author, “hands-free” extractors
such as the artas robots and vacuum suction are fraught with
to many potential problems to be the answer for your FUE
hair transplant. No automated device can discern a prime
follicular unit as an experienced physician can, nor extract
it with the care required to preserve its life for
implantation into the recipient area. While the NeoGraft and
Artas are both faulty for this obvious reason, they also
each carry individual problems.
NeoGraft:
Uses an air vacuum to extract donor follicles. This
very mechanism opposes the nature of FUE hair
transplant, as it dries out the follicle, potentially
killing it.
When used to implant grafts, it would not
allow follicles to be tightly packed into the recipient
area, as the implantation device is too large for such
specificity.
The vacuum sucked follicle on its way to the
receiving bottle will collide violently against
the tubing of this device, further harming it and
compromising its survival.
If used to place grafts in the recipient would
increase the risk of buried grafts due to the relative
loss of depth control. This leads to cysts and pitting
on the scalp.
Artas:
Higher transection rates of up to 15% has been cited
for this device although its manufacturer’s and vendors
claim the transection rate is improving with time, this
is difficult to ascertain at this point. In the hands of
an experienced FUE practitioner a transection rate
of 2-5% is common.
Can only be used on people with straight dark brown
or black hair.
Since it uses a sharp punch followed by a dull
punch, there is an increased risk of buried grafts s the
dull punch may push the partially separated grafts
furthe into the scalp increasing the risk of cyst
formation, not to mention the graft that is damaged in
the process.
Since the machine would have punch out hundreds of
grafts before the physician typical check in, if
transection rates are high, many grafts would have been
wasted before the procedure is aborted.
It is a cumbersome process. The patient is placed in
a particular position while an area of about 5 by 10 cm
is isolated for the machine to gauge. The distance from
the machine is set. The machine then goes to work.
obviously any movement on the patients path would be
problematic. After this area is worked, the physician
then removes the grafts with a forcep. Then the process
of strapping the patients head in a different position
and machine calibration is done all over again. The
procedure is clearly too cumbersome for the patient and
not efficient when compared to an experienced FUE
surgeon using a hand held device to feel around each and
every grafts as it is removed. Control is by the seconds
rather than between hundreds of graft separation.
FUE HAIR TRANSPLANTATION CONTINUES TO PROVE ITS
VALUE
The FUE hair transplant is, hands down, today’s leading
surgical technique for hair restoration. While it does
require practitioners to take their time implementing the
utmost skill, the life-changing results patients enjoy makes
it a highly rewarding process overall. FUE hair transplant
using body hair transplantation has also become much more
widely used and accepted because of its undeniable success
rates in the hands of experienced practitioners
Growth of transplanted hair
after surgery
The transplanted hair grow for 2-3 weeks, then the hair shafts
fall off, the roots remaining in the scalp and the shafts start
coming back after 2-3 months of surgery.
The hair come back at a very slow pace and a great deal of
patience is required during this period. About 40-50% hair come back
after 6 months and 75% after 9 months and about 90% of transplanted
hair come back after 1 year of surgery.
FUE Post OP Instructions
After the surgery, you will have minimal discomfort but not that
much to keep you awake all night. It is advisable to lie down in
supine position and keep the towel at the back of your head while
sleeping.
Painkillers will be prescribed which you would require for first
3-4 days only.
Please do the forehead massage starting from the middle of your
head towards outside 10- 15 times a hour or 100-150 times in a day
for 5 days.
Please remove the back bandage (of donor area) on the 3rd day
after surgery. You can do this yourself. After removal start
applying antibiotic cream for 7 days.
Bandages on the forehead must not be removed till the swelling
goes off(may be 5-6 days).
Eat a healthy diet - ( high protein )
Non - Vegetarian :- Fish, chicken, eggs
Vegetarian :- Soyabean, paneer, daals, almonds, (with skin, 5-6
daily)
Avoid direct sun light, sweat for 2-3 weeks. No upper body
exercise/ heavy exercises (Gym) till 3 weeks.
You can follow your routine activity from the next day onwards.
Please use the spray to moisten the transplanted area, every 2
hourly for the first 2 days, then 4 hourly for next 3 days.
You can have shower on 5th day without hands, without shampoo.
On 7th day, have shower with baby shampoo, but no hands to be
used.
On 10th day onwards, normal shower with baby shampoo can be done
by using hands.
Microfollicular Hair Transplant Procedure
Hair transplant is a comfortable procedure performed
as a Day Care procedure, performed under local
anesthesia,while the patient may work on his laptop,
mobile or simply read a novel. After the numbing effect
of the local anesthesia has taken place.
A strip of hair bearing skin is taken from the back of
the head. This area is relatively unaffected by the male
hormones which are responsible for baldness. The hair
bearing strip is separated to single follicular units
under magnification. It is to be noted that each
follicular unit contains more than 1 follicle. A good
team performing hair transplant manages each unit to
contain 1-4 hair follicles. On an average each graft or
a follicular unit contain 2.5 hair follicles or hair.
While the assistants are separating grafts from
the strip, the surgeon makes micro slits in the area of
the transplant. This step is the most important step
since the surgeon decides the density of the transplant,
and the direction of the transplanted hair at this
stage. The next step is implantation of the grafts into
these pre formed slits. This procedure can take 3- 6
hours depending on the number of grafts. Usually these
two procedures are done side by side, so the whole
process can be expected to last 4-6 hours. For the front
of the head about 1500-2000 hair follicle units are
required. For a complete coverage, about 6000 units may
be required. It is preferable to make a second
replacement after 6-8 months.For 1000-1200 grafts, it
takes 3 hours, while a 2000-2200 graft session takes 4.5
hours.
Hair transplant for
bald areas
This involved
hair transplant in areas where there is no hair,
and that patch is effectively bald. Extremely
satisfying results are obtained in this group.
Hair transplant for
density increase
This is more
difficult, since any injury to existing hair is
to be avoided. Extremely satisfying results are
achieved. The instrumentation for this procedure
is slightly different.
Pre Procedure
Protocol
Patients are
required to complete a Consult Form ( ATTACHMENT

particularly anti diabetics, blood thinner
medicines must be revealed.
All patients should do the following tests –
Haemoglobin, blood sugar, blood urea, HIV,
HbsAg, Anti HCV. For patients above 35yrs, ECG
is also advised.
Rationale
The hair is taken
from the back portion of the head, which is
relatively resistant to action of male hormone
responsible for hair loss. Thus this hair
persists nearly life long. The hair that is
transplanted carries with it the genetic
property of its original position, so it resists
hair fall.
The techniques for hair transplant are -
Microfollicular hair transplant with
Strip procedure.
Follicular hair transplant – extraction
of roots one by one Body hair transplant –
use of hair from the chest, arms, pubic
area, underarms etc.
Refinements
Hair
transplant has undergone a number of
refinements. In the microfollicular technique,
the earlier used punches have given way to use
of micrografts. These are follicular units with
1-3 hair follicles, giving a very Natural hair
line appearance. The use of fine and delicate
instruments like microneedles, micropunches,
micro forceps etc have made handling of tissues
more delicate and with no damage to hair
follicles at all.
Use of microscopes and magnification has allowed
larger hair follicle harvesting and better
survival of grafts.
Long acting anesthetic agents, allow long term
pain relief, allowing the procedure to be more
comfortable.
Post Transplant Care
We advise
the following medicines and care after a
transplant -
Tablet levoflox 750 mg once daily for 5
days.
Tablet ketanov thrice daily for 5 days.
Nupatch 100 mg twice daily for 5 days.
Please keep the head band on for 3 days.
The purpose of the head band is to reduce
the swelling that occurs in the forehead
region.
Head washing is to be started on the 4th
day after removing the head band. The
washing is to be gentle over the
transplanted region, and scrub over the area
of the stitches to keep them clean. This
needs to be done daily from the 4th – 18 the
day

AFTER HAIR TRANSPLANT SURGERY
What is the after care of hair
transplant surgery? Post-operative
instructions.
You will leave the hair transplant
clinic on the same day with no bandages
on - but you should wear a cap for a
week to protect grafted from dust, sun &
heat.
Take medications as directed. (Avoid
Aspirin for 2 weeks)
Sleep in the position in which
you're most comfortable but sleep with
the head on 2-3 pillows for a few
nights.
Place wrapped ice pack on your
forehead & eyes but not on the graft
sites for 48 hours to avoid swelling.
Swelling will go away in a few days of

swelling is uncommon due to medications
taken post operatively.
After 6 hours of the hair transplant
surgery shampoo the graft site & donor
site gently without rubbing and wash the
head with shower. Blow dry or dry
without rubbing the graft site until all
scabs fall out.
If bleeding occurs apply a light &
steady pressure for 20-30 minutes with a
clean cloth at the site of bleeding.

dissolved & need not be removed.

Why Hair
Transplant Works?
Hair in the front and top of head is lost
because it is Genetically hormone susceptible.
Hair on the sides and lower back are genetically
immune to hormone sensitivity and are
genetically programmed to grow a lifetime, it
will continue to grow ven though it is
transplanted to a different site. This is the
medical fact, and has been repeatedly proven for
more than 50 years.
Follicular Unit Hair Transplant is the only
method by which one can grow permanently
growing, natural looking, and undetectable hair
in the bald or thin area. In olden days people
used Punch or plug grafts where bunches of 15-25
hair in a plug taken from the side or the back
of the scalp (Donor Area) were implanted in the
front. This gave the typical pluggy (Cornrow,
baby doll, tooth brush or cobble stone)

They were later replaced in 1980s by Mini and
then Micro grafts, which were randomly, cut

Alopecia Areata:
This as a common Autoimmune
disease where the body 's immune
system perceives the hair as its
own enemy and start attacking
and destroying them. This is
treatable only in early stage
but recurrence may not be
prevented. It may recur in other
part of scalp, beard or other
part of scalp. Chronic disease
is difficult to cure when many
roots are destroyed. Hair

Scarring or Cicatricle Alopecia:
After burns or other injuries to

If you have Androgenetic Alopecia, like many
other people, you were destined to lose hair
from the time you were conceived. You have a
group of genes that act together to make
your hair susceptible to natural hormones in
the body. You inherited these genes from
either side of the family or from both sides
and these genes may affect you more or less
than other members of the family.
Androgens, the so-called male hormones, are
produced by both men and women, in the
testicles, ovaries, and adrenal glands. The
adrenal glands are small glands above the
kidneys. Testosterone is the most well-known
androgen. At puberty, testosterone levels
rise and, among other effects, cause the
fine, short, "velus" or baby hair in the
pubic areas and in the armpits to become
coarser hair.
In the scalp, testosterone is converted, by
an enzyme called "5-alpha reductase", to a
different androgen called
"Dihydrotestosterone (DHT)." DHT, more than
any other androgen, causes genetically
sensitive hair to be lost. In genetically
sensitive hair, DHT causes each generation
of hair to become progressively smaller.
This process is called miniaturization. DHT
causes each generation of hair to become
more narrow, short and lighter in color.
Eventually the root stops producing hairs
altogether.
There are two medications that have been
proven to slow down Androgenetic Alopecia.
Finasteride (Propecia or Proscar) blocks a
type of five alpha reductase to decrease the
amount of DHT. Minoxidil (Rogaine) seems to
work directly on the hair cells and prolongs
the growing phase. Medications can slow down
the process on a temporary basis but they
cannot stop it. Follicular unit hair
transplantation, however, can provide a
lasting improvement.
Some hairstyles, like braiding can cause
hair loss.
Styles that pull or put tension on the hairs
- such as tight ponytails or cornrows - can
cause hair loss.
Hair loss can be aggravated by other
conditions.
Iron deficiency can make hair loss worse and
is fairly common in vegetarians and women.
Iron deficiency is not a diagnosis by itself
and the cause must be found. Correcting iron
deficiency can slow down the progression of
hair loss but is not likely to cause a major
improvement in your hair. Severe
malnutrition and protein deficiency can also
contribute to hair loss but hair loss would
be the least of your worries. Thyroid
disease can aggravate hair loss. Hair loss
can occur with either an under active
thyroid or an overactive thyroid. Elevated

Both psychological and physical stress,

What is Hair
Transplantation?
Hair transplantation is a surgical procedure
in which hair from the back and sides of the scalp is moved to

When Can a Hair Transplant Be Done?

the later stages when baldness is more apparent.

How Long Will the Transplanted Hair
Last?
Transplanted hair should grow normally in the

Do All Transplanted Hair Grafts Grow?

postoperative instructions are provided to minimize the risk of
graft injury and subsequent compromised growth. It is
important to realize that the number of hairs growing from each
graft will vary, depending on the density of the hair at the
donor site and on the size of the graft selected.
How is this Surgical Hair Restoration
Procedure Performed?
Hair Transplantation is a long office
procedure, which requires only local anesthetic. There is
initially some discomfort associated with the administration of
the anesthesia, but this is of very short duration. We have
developed techniques to reduce this discomfort, and most
patients tolerate this brief period very well.
The donor area (the back or sides of the scalp) as well as
the recipient area (the bald or thinning area) are made numb
with the local anesthetic. Up until several years ago,
round grafts (plugs) were always used throughout the hair
transplant process. The desired number of round grafts were
removed from the donor area and then transferred to the bald
area. Today these larger grafts have been mostly replaced
by smaller grafts (follicular units, micro, mini or slit
grafts), since these grafts produce a much more natural
transition from thinning or baldness to increased hair
growth. In most cases where very small grafts are used,
this surgery can be accomplished almost undetectably.
Specific recommendations are discussed at a personal
consultation.
When smaller grafts are to be transplanted, the
following technique is used:
The hair to be transplanted is trimmed close to the scalp,
and the desired amount of donor skin and hair are then
removed. This donor site is then sutured closed, resulting
in a fine scar which is only detectable on close
inspection. The donor hair-bearing skin is then divided
into very small pieces which generally contain 1-3 hairs.
These grafts are then rapidly inserted into small openings
made in the recipient area.
When there is a Large Area of Baldness,
is it Possible to cover the Entire Bald Area?
This depends on the size of the bald area as well as on the
donor area. When there is extensive baldness, a procedure
called a scalp reduction may be useful in order to decrease
the size of the bald area. Subsequently, grafting is done
as described above.

FUE Post OP Instructions
After the surgery, you will have minimal discomfort but not that
much to keep you awake all night. It is advisable to lie down in
supine position and keep the towel at the back of your head while
sleeping.
Painkillers will be prescribed which you would require for first
3-4 days only.
Please do the forehead massage starting from the middle of your
head towards outside 10- 15 times a hour or 100-150 times in a day
for 5 days.
Please remove the back bandage (of donor area) on the 3rd day
after surgery. You can do this yourself. After removal start
applying antibiotic cream for 7 days.
Bandages on the forehead must not be removed till the swelling
goes off(may be 5-6 days).
Eat a healthy diet - ( high protein )

Avoid direct sun light, sweat for 2-3 weeks. No upper body

You can have shower on 5th day without hands, without shampoo.
On 7th day, have shower with baby shampoo, but no hands to be

Microfollicular Hair Transplant Procedure
Hair transplant is a comfortable procedure performed
as a Day Care procedure, performed under local
anesthesia,while the patient may work on his laptop,
mobile or simply read a novel. After the numbing effect
of the local anesthesia has taken place.
A strip of hair bearing skin is taken from the back of
the head. This area is relatively unaffected by the male
hormones which are responsible for baldness. The hair
bearing strip is separated to single follicular units
under magnification. It is to be noted that each
follicular unit contains more than 1 follicle. A good
team performing hair transplant manages each unit to
contain 1-4 hair follicles. On an average each graft or
a follicular unit contain 2.5 hair follicles or hair.
While the assistants are separating grafts from
the strip, the surgeon makes micro slits in the area of
the transplant. This step is the most important step
since the surgeon decides the density of the transplant,
and the direction of the transplanted hair at this
stage. The next step is implantation of the grafts into
these pre formed slits. This procedure can take 3- 6
hours depending on the number of grafts. Usually these
two procedures are done side by side, so the whole
process can be expected to last 4-6 hours. For the front
of the head about 1500-2000 hair follicle units are
required. For a complete coverage, about 6000 units may
be required. It is preferable to make a second

Post Transplant Care
We advise
the following medicines and care after a
transplant -
Tablet levoflox 750 mg once daily for 5
days.
Tablet ketanov thrice daily for 5 days.

Please keep the head band on for 3 days.
The purpose of the head band is to reduce
the swelling that occurs in the forehead
region.
Head washing is to be started on the 4th
day after removing the head band. The
washing is to be gentle over the
transplanted region, and scrub over the area

needs to be done daily from the 4th – 18 the

You will notice scabs around each hair

well-trained team of over 8 people. Dense packing consumes many
more grafts because one sq. c.m. is implanted with 25 to 50
grafts. This is normally done in the area of frontal forelock so
as to have fullness in the front without exhausting the donor

AFTER HAIR TRANSPLANT SURGERY
What is the after care of hair transplant surgery?
Post-operative instructions.
You will leave the hair transplant clinic on the same day
with no bandages on - but you should wear a cap for a week to
protect grafted from dust, sun & heat.
Take medications as directed. (Avoid Aspirin for 2 weeks)
Sleep in the position in which you're most comfortable but
sleep with the head on 2-3 pillows for a few nights.
Place wrapped ice pack on your forehead & eyes but not on
the graft sites for 48 hours to avoid swelling. Swelling will go
away in a few days of the hair transplant surgery. These days
swelling is uncommon due to medications taken post operatively.
After 6 hours of the hair transplant surgery shampoo the
graft site & donor site gently without rubbing and wash the head
with shower. Blow dry or dry without rubbing the graft site
until all scabs fall out.
If bleeding occurs apply a light & steady pressure for 20-30
minutes with a clean cloth at the site of bleeding.
Sutures in the donor area gets dissolved & need not be
removed.
Avoid alcohol for 48 hours after hair transplant surgery.
Avoid smoking for as long as you can after hair transplant
surgery.
No swimming in chlorinated water for 2 weeks after hair
transplant surgery.
Rest for 1 day after hair transplant surgery.
Light work for a week after hair transplant surgery.
Heavy work, exercise or sex after 2 weeks of hair transplant
surgery.
Visit your hair dresser for hair cut after 3 weeks & for
hair styling after 6 weeks of hair transplant surgery.
Diet: Healthy nutritious diet with more proteins is
beneficial. A diet that contains high quantities of animal fat
can lead to an increase concentration of testosterone in the
blood stream, increasing your risk of hair loss. High protein
diet in pulses, milk, and white of egg, red meet and fish are
good. Green vegetables and fruits are good source of minerals
and antioxidants. A lot of water keeps your hair texture good.
Cold laser for 3-6 weeks if prescribed.
You may be receiving medications during the hair transplant
surgery that can make you drowsy. Because of this we strongly
recommend the not to drive home the day of your hair transplant
surgery. If you are unable to have someone drive you home after
your hair transplant surgery and you live near our office we can
arrange to have you driven home. If you are coming from far we
will arrange for lodging at a hotel near our office so that you
may stay overnight.
Report to the hair transplant clinic or call the hair
transplant surgeon if:
any excessive pain or excessive bleeding
any rise in temperature over – 100oF
sudden swelling of forehead or eyes
How will the result of hair transplant surgery look?
Advantage of follicular hair transplant surgery is that the
result is not only natural looking and natural feeling but hair

If hair transplant surgery is a day treatment will I have
full-grown hair when I leave the hair transplant clinic?
The treatment of grafting the hair roots is over in few hour
of the day and you may have short hair sticking out from the

You don't need any special care for the newly grown hair

You may have tightness in the stitch area for a few days
after hair transplant surgery, but soon it disappears.

You will have a thin line of scar from where we remove the
strip but rest assured it will remain undetectable because it
will be covered under the hair on the top.
Do I need to remove the hair transplant surgery stitches?
Usually the hair transplant surgery stitches get dissolved
by 12-15 days.
Do I need to take the medication and for how long after hair
transplant surgery?
Usually we prescribe antibiotics for a week and painkillers
for 5 days after hair transplant surgery. Additionally you may
take finasteride and minoxidil for thinning hair or preventing
hair fall by telogen effluvium.
Do you use laser for Hair Transplant Surgery? What is Low
Level Laser Therapy or Cold Laser?
Few years back hair transplant community used laser to drill
micro holes for implanting hair roots. They soon found out that
the hair growth was poor and laser was doing more damage to the
wall of the hole and its blood supply then it helped. As a
result no hair transplant surgeon of good standing in the world
uses laser for hair transplant. But for Low Level Laser Therapy
(lll) or Cold Laser as it is called is proclaimed promising for
improving the thinned out hair harmlessly are used by us after
hair transplant surgery to enhance the results.
How soon can the same hair transplant surgery be repeated in
the same area?
Theoretically, to increase the density one may repeat the
plantation within 6-8 months of the hair transplant surgery.
Why Hair Transplant Works?
Hair in the front and top of head is lost because it is Genetically
hormone susceptible. Hair on the sides and lower back are genetically
immune to hormone sensitivity and are genetically programmed to grow a
lifetime, it will continue to grow ven though it is transplanted to a
different site. This is the medical fact, and has been repeatedly proven
for more than 50 years.
Follicular Unit Hair Transplant is the only method by which one can grow
permanently growing, natural looking, and undetectable hair in the bald
or thin area. In olden days people used Punch or plug grafts where
bunches of 15-25 hair in a plug taken from the side or the back of the
scalp (Donor Area) were implanted in the front. This gave the typical
pluggy (Cornrow, baby doll, tooth brush or cobble stone) appearance.
They were later replaced in 1980s by Mini and then Micro grafts, which
were randomly, cut smaller grafts of 5-8 hairs and 1-3 hairs
respectively. The results improved but were still not near natural
looking. 1983 it was discovered that nature grows our hair in groups of
1,2,3 or 4 hairs. These were called the Follicular Units. So surgeons

Description
Common Baldness
(MPB or AHA) and FPB Androgenetic Alopecia (AHA) is by far
the most common cause of hair loss. This is due to 2 reasons
1. Genes
2. Hormones
Alopecia Areata:
This as a common Autoimmune disease where the body 's
immune system perceives the hair as its own enemy and
start attacking and destroying them. This is treatable
only in early stage but recurrence may not be prevented.
It may recur in other part of scalp, beard or other part
of scalp. Chronic disease is difficult to cure when many
roots are destroyed. Hair Transplant is usually not
done.
Scarring or Cicatricle Alopecia:
After burns or other injuries to the scalp there can be
permanent damage to the hair roots and in that patch
hair do not grow. Usually due to scarring the blood
supply in that area is poor.
Traction Alopecia & Trichotelomania:
Due to the tight hair style in women or in Sikhs like
braiding or the use of weave or pins of wig continually
pulls on the hair and the roots may get damaged in long
run and can cause hair loss. In Obsessive psychiatric
disorder called Trichotellomania the patient is in a
habit of pulling the hair in one area and ends up in a
patchy hair loss in that area.
Telogen Effluvium:
A period of Physical or mental stress, delivery of a
baby in females etc. can lead to generalized loss of
hair from scalp within few weeks due to shortening of
Growth cycle and entering of hair in Telogen phase. This
is temporary because after 6 months of relief in stress
period the hair starts regrowing. Telogen effluvium can
precipitate or even aggravate pattern baldness if one is
genetically susceptible to develop MPB or FPB
Anagen Effluvium:
Chemotherapy or certain Toxins can stop further growth
of the growing hair and the already grown hair brakes
and sheds over few days.
If you have Androgenetic Alopecia, like many other people, you were
destined to lose hair from the time you were conceived. You have a
group of genes that act together to make your hair susceptible to
natural hormones in the body. You inherited these genes from either
side of the family or from both sides and these genes may affect you
more or less than other members of the family.
Androgens, the so-called male hormones, are produced by both men and
women, in the testicles, ovaries, and adrenal glands. The adrenal
glands are small glands above the kidneys. Testosterone is the most
well-known androgen. At puberty, testosterone levels rise and, among
other effects, cause the fine, short, "velus" or baby hair in the
pubic areas and in the armpits to become coarser hair.
In the scalp, testosterone is converted, by an enzyme called
"5-alpha reductase", to a different androgen called
"Dihydrotestosterone (DHT)." DHT, more than any other androgen,
causes genetically sensitive hair to be lost. In genetically
sensitive hair, DHT causes each generation of hair to become
progressively smaller. This process is called miniaturization. DHT
causes each generation of hair to become more narrow, short and
lighter in color. Eventually the root stops producing hairs
altogether.
There are two medications that have been proven to slow down
Androgenetic Alopecia. Finasteride (Propecia or Proscar) blocks a
type of five alpha reductase to decrease the amount of DHT.
Minoxidil (Rogaine) seems to work directly on the hair cells and
prolongs the growing phase. Medications can slow down the process on
a temporary basis but they cannot stop it. Follicular unit hair
transplantation, however, can provide a lasting improvement.
Some hairstyles, like braiding can cause hair loss.
Styles that pull or put tension on the hairs - such as tight
ponytails or cornrows - can cause hair loss.
Hair loss can be aggravated by other conditions.
Iron deficiency can make hair loss worse and is fairly common in
vegetarians and women. Iron deficiency is not a diagnosis by itself
and the cause must be found. Correcting iron deficiency can slow
down the progression of hair loss but is not likely to cause a major
improvement in your hair. Severe malnutrition and protein deficiency
can also contribute to hair loss but hair loss would be the least of
your worries. Thyroid disease can aggravate hair loss. Hair loss can
occur with either an under active thyroid or an overactive thyroid.
Elevated Hormones can speed up hair loss. Men (or women) who take
anabolic steroids for bodybuilding often notice accelerated hair
loss.
Does stress contribute to hair loss?
Both psychological and physical stress, especially if they are
intense and prolonged, can contribute to temporary hair loss. They
do this by perhaps altering the hair cycle: shortening the growing
stage or lengthening the resting stage. Stress may also have a
negative impact on the immune system leading to a more severe type
of hair loss called alopecia areata. When the stressful situation is
over or the body adjusts to it, the hair usually grows back.
